Document Checklist

These are starter checklists. Only provide documents actually requested by an official program, carrier, marketplace, or qualified professional.

Individuals and families

  • Names and dates of birth for household members
  • New York county or NYC borough
  • Current address or proof of New York residence if requested
  • Income estimate and employer coverage information
  • Any renewal, denial, or request-for-documents notice

Complex or mixed households

  • Separate who needs coverage from who is only part of the household
  • Use plain-language status categories only when needed
  • Do not upload full immigration documents unless specifically requested

Small business

  • Business legal name and address
  • Eligible employee count and census without SSNs
  • Desired effective date and contribution goal
  • Current coverage and current broker if any
